Where these numbers come from

They are pulled from the mode's own files for patch 26.16, the one currently live. The source is community-run rather than an official Riot API, and the game shifts with community votes, so a gap with your own game is always possible.

Nightmare

He pokes you from outside your reach, blinds you the moment you try anything, and his mushrooms make moving around a chore. You are not going to touch him in lane, so stop trying. Your only goal is the counter: sit under tower, collect every last hit you can, and buy magic resist rather than armor. Get your jungler in early, because a Teemo without vision is a dead Teemo, and remember that late in the game he can't do anything to you at all.
