They are pulled from the mode's own files for patch 26.15, the one currently live. The source is community-run rather than an official Riot API, and the game shifts with community votes, so a gap with your own game is always possible.

In human form, Nidalee throws a spiked javelin at her target that gains damage as it flies. As a cougar, her next attack will attempt to fatally wound her target, dealing more damage the less life they have.

Human: Nidalee transforms into a vicious cougar, gaining 20 Movement Speed, and her basic abilities become Takedown, Pounce, and Swipe. Cougar: Nidalee transforms back into human form.
In short
Nidalee is the only champion on this roster who swaps her whole kit mid-fight. In human form she throws a javelin that hits harder the further it travels, plants traps that reveal and strip resistances, and heals an ally while handing them attack speed. In Aspect of the Cougar she loses all of that and gains a leap, an area claw swipe, and a Takedown that hits harder the lower her target already is.
The champion rewards precision and nothing else. A spear landed at full distance decides the lane, a missed spear costs almost nothing and gains exactly as much. The transformation is free and comes back fast, so the classic mistake is not playing a form badly, it is standing in the wrong one. She is fragile, she has no crowd control whatsoever, and she survives purely on movement.
Strengths and weaknesses
Her javelin reaches well past standard mage range, letting her poke over a wall, out of a bush or through a jungle entrance without ever showing herself. Prowl gives her speed the moment she moves through tall grass, which makes her jungle routes and her ambushes unusually quick. Takedown in cougar form executes a low target better than any other tool on the roster, and Primal Surge keeps her and her allies topped up between trades, which makes her miserable to grind down.
The flaws are just as readable. She has no way at all to root or slow an enemy, so anyone who decides to leave simply leaves. Her spear travels slowly and gets eaten by a minion, which makes her harmless against someone who stands in their wave. Swapping to cougar means walking into melee range with a mage's body, and silence is her nightmare, because it locks her into whichever form she happens to be in.
Laning phase
Top lane, you play the distance. Hug the side of the corridor, look for the angle that gives your javelin the longest possible flight, and never walk forward to throw: the further it travels, the harder it lands. Drop a trap in the bush your opponent has to walk past on his way to farm, it reveals him and melts his resistances, which turns an average spear into a won trade.
Do not swap to cougar on reflex. There are three reasons to do it: finishing a low opponent with Takedown, escaping a gank with a leap over terrain, or clearing a wave in a hurry. The rest of the time you are trading away your spear and your heal for a melee fight your stat line cannot win. Primal Surge in human form is also your lane sustain: cast it on yourself between waves instead of walking back to base.
Power spikes
The first spike is your Aspect of the Cougar level. It is not a damage spike, it is a survivability and threat spike: from then on, a failed gank on you becomes a double mistake, because you leap over terrain and can come back to finish whoever is hurt. It is also the point where your lane starts generating pressure on the lanes next door.
The second spike arrives with your first ability power item paired with a source of slow. Nidalee chases nobody on her own, but the moment she can slow a target, the spear connects and the cougar sequence closes it out. From there, every enemy crossing the map at half health is a corpse on delay, and your role shifts from lane bully to map-wide hunter.
Teamfights and positioning
In a ranged fight you stay human, stay far back, and throw spears into the pile. It is not glamorous but it is correct: your range is your only edge against a grouped team, and you do not last ten seconds inside it. Primal Surge on your carry is often worth more than the damage you would have added by stepping forward.
You go cougar only to collect an isolated target peeling off the fight, never to open. The sequence to repeat is simple: spear from range, swap once the target is hurt, leap to close, Takedown to finish. Before the fight, your traps do the invisible work: placed around baron and on jungle entrances, they reveal groupings and weaken whoever walks over them.
